{"id":558,"date":"2018-03-28T13:46:23","date_gmt":"2018-03-28T17:46:23","guid":{"rendered":"https:\/\/www.joannhoffman.com\/blog\/?p=558"},"modified":"2018-03-28T13:46:23","modified_gmt":"2018-03-28T17:46:23","slug":"making-dba-claim-completing-earning-form","status":"publish","type":"post","link":"https:\/\/www.joannhoffman.com\/blog\/making-dba-claim-completing-earning-form\/","title":{"rendered":"Making a DBA Claim and Completing the Earning Form"},"content":{"rendered":"<p>For those workers who have been sent to war zones to work, you may discover you have injuries once you return to your home even if in a different country. \u00a0When you are having difficulty hearing from being exposed to bombing or experiencing nightmares or startle responses to loud noises, you should seek medical treatment in your own country. \u00a0Until you see a doctor we cannot make a claim for you. \u00a0Once you see the doctor we will need to see those medical records. \u00a0We can then file a Claim and I discuss that in my other blogs.<\/p>\n<p><a href=\"http:\/\/www.joannhoffman.com\">This firm<\/a> represents many injured workers who have lost wages since their last employment when they were under contract to work. I want to provide access to the form you will need to complete to have your lost wages considered. The form is called an <a href=\"https:\/\/www.dol.gov\/owcp\/dlhwc\/ls-200.pdf\">LS200 Report of Earnings<\/a>. What it does is allow the injured worker to document his earnings since the time he lost his employment where his injury is ongoing as with spinal physical injuries or psychiatric injuries. \u00a0This is important because this documentation allows the insurance company to calculate the wage loss of the injured worker.<\/p>\n<p style=\"text-align: left;\"><img loading=\"lazy\" decoding=\"async\" class=\"size-thumbnail wp-image-562 alignleft\" src=\"https:\/\/www.joannhoffman.com\/blog\/wp-content\/uploads\/2018\/03\/IMG_4604-e1522095418631-1-150x150.jpg\" alt=\"IMG_4604-e1522095418631-1-150x150\" width=\"150\" height=\"150\" \/>Let me explain it with an example. Say when you were working you earned $200 per week. \u00a0Your psychiatric injuries have prevented you from working for 6 months of the twelve months since you lost your employment.
